Verification of Family Relationship for Sperm and Egg Donation:

Taiwanese Residents:

When artificial reproduction institutions use donated eggs or sperm for artificial reproduction, they should first issue a "diagnostic certificate" to the recipient couple. The couple should then take the certificate to the local household registration office, where the staff will assist you in obtaining the wife's second-degree relative relationship chart and the husband's fourth-degree relative relationship chart.

Both spouses must personally go to the court or a private notary office to notarize the “Egg Donation Consent Form”.

Once this process is completed, the PCFC will help you handle the rest of the administrative procedures.

Mainland China, Hong Kong, Macau, and Foreign Nationals:

If one spouse is a foreign national and encounters difficulty obtaining proof of family relationship, they can return to the medical institution and fill out a form for proof of family relationship up to the fourth degree of kinship (we will assist you in filling out the form). They should also complete a "declaration" explaining the difficulty of obtaining family information as a foreign national. Both spouses must sign this declaration. The PCFC will then handle all administrative procedures.
